What the Motor Vehicles Act Says About Form 1A Doctors

Form 1A is the medical fitness certificate required under Rule 5 of the Central Motor Vehicles Rules 1989, made under the Motor Vehicles Act 1988. The relevant provision states that the medical certificate must be issued by a “registered medical practitioner” — defined in Section 2(25) of the Motor Vehicles Act as a person registered under any central or state law regulating the registration of medical practitioners.

Under the National Medical Commission Act 2019 (which replaced the Indian Medical Council Act), the NMC maintains the register of all licensed medical practitioners in India. Any doctor with an active NMC registration — i.e., any MBBS or MD doctor with a valid NMC registration number — is a “registered medical practitioner” under the Motor Vehicles Act and is legally authorised to issue Form 1A.

Why Do People Search for “RTO-Approved Doctors”?

The confusion arises because some state RTOs — particularly in Maharashtra, Uttar Pradesh, and a few other states — maintain an informal or unofficial “empanelled doctor” list as part of their internal RTO process. This is not a requirement under central law. It is an administrative convenience some RTOs use for in-person medical examinations conducted at the RTO premises.

However, when you submit a Form 1A certificate obtained from any NMC-registered doctor (in-person or online), the RTO is legally required to accept it if the certificate contains all mandatory elements — the doctor's NMC registration number, signature, clinic stamp, vision assessment, hearing assessment, and general fitness declaration. RTOs that reject valid Form 1A certificates from NMC-registered doctors exceed their legal authority.

Can Form 1A Be Issued Online by a Telemedicine Doctor?

Yes. Under the MoHFW Telemedicine Practice Guidelines 2020 (GSR 225(E), Gazette of India, 25 March 2020), NMC-registered doctors are authorised to conduct medical assessments and issue certificates including Form 1A via telemedicine. The Motor Vehicles Act requires the certificate to be from a registered medical practitioner — it does not specify that the examination must be in-person.
