Go to parivahan.gov.in → Sarathi Services, select your state, and choose your service: learner licence, DL renewal, international permit, or another DL service.

Enter your personal and licence details and pay the statutory fee. The portal generates your application number — keep it, you will need it at the upload step.

Apply here, pay ₹999, and an NMC-registered MBBS/MD doctor completes the vision, hearing and general fitness assessment online, then signs and stamps the form.

Your completed Form 1A arrives on WhatsApp and email within 30 minutes as a ready-to-upload PDF, carrying the doctor’s name, NMC registration number, signature and stamp.

Return to your Sarathi application, open the medical certificate section, and upload the signed PDF. You do this yourself — we send the certificate to you rather than filing it on your behalf.

The Regional Transport Office checks the certificate and can verify the doctor’s NMC registration number independently at nmc.org.in. Some states also require an in-person biometric or document-verification visit.

Our Form 1A is issued by NMC-registered MBBS/MD doctors, and the NMC registration number printed on the certificate is independently verifiable at nmc.org.in. Some states run an empanelment system where a doctor registered on the Parivahan/Sarathi portal can file Form 1A directly against your application; in the rest of India the applicant uploads the certificate themselves. We issue the signed certificate and send it to you to upload — we do not file it directly against your application in any state. Karnataka, Maharashtra, Tamil Nadu and Odisha require a portal-empanelled doctor, so we do not currently serve those four states.

Open the Parivahan downloadable forms page for Central Motor Vehicles Rules (CMVR) forms at parivahan.gov.in and find "Form 1A — Medical Certificate", or go directly to parivahan.gov.in/sites/default/files/DownloadForm/cmvr/FORM-1A.pdf. The blank form is free and needs no login. You can also download the same standard CMVR 1989 format directly from this page. Remember that only your basic details may be self-filled: the medical sections must be completed, signed, and stamped by a registered medical practitioner, or the RTO will reject it. If you get your Form 1A online instead, you do not need the blank form at all — the doctor sends a completed, signed copy ready to upload.

A Form 1A medical certificate is valid for six months from the date of issue. You must submit it to the RTO or upload it to the Parivahan portal within that window — if it lapses before your driving licence application is processed, the RTO will ask for a fresh certificate. Form 1A cannot be reused for a later renewal cycle, so commercial and transport licence holders need a new one at each renewal regardless of when the previous certificate was issued.

Log in at parivahan.gov.in, open your driving licence application or renewal under Sarathi Services, and go to the document upload step. Select the medical certificate document type and upload your Form 1A PDF. The portal accepts PDF certificates issued by NMC-registered doctors, including digitally signed ones. Keep the file under the portal size limit (usually 200 KB–1 MB depending on your state) — the PDF we send is already within that limit and needs no compression or printing.

Form 1A rejections are almost always down to paperwork rather than health: a missing NMC registration number, an unsigned or unstamped form, details that don't match your licence application, or an expired certificate past the six-month window. Every certificate we issue carries the doctor's full name, NMC registration number, signature, and stamp, and the number is independently verifiable at nmc.org.in. If an RTO raises a query about a certificate we issued, contact us and we will correct and reissue it at no charge.

Yes. Under the MoHFW Telemedicine Practice Guidelines 2020, NMC-registered doctors are authorised to conduct online assessments and issue Form 1A medical certificates. The certificate includes the doctor's NMC registration number, which RTO officers can verify at nmc.org.in. Our certificates are accepted at RTOs across all states and union territories in India.

Our Form 1A online assessment covers: (1) Visual acuity test — checking eyesight for both eyes with or without corrective lenses, (2) Colour vision test — identifying traffic signal colours, (3) Hearing/ear assessment — checking auditory response to normal and loud sounds, (4) General fitness assessment — cardiovascular fitness, blood pressure, and general health status. The assessment is conducted by an NMC-registered MBBS or MD doctor.

Most Form 1A certificates are delivered to your WhatsApp and email within 30 minutes of completing the online assessment and payment. During peak hours it may take up to 60 minutes. Service runs 8 AM to 10 PM IST, 7 days a week including public holidays and Sundays.

Form 1A is required for both: (1) Fresh driving licence applications — all first-time applicants must submit Form 1A, (2) Driving licence renewal — required when your licence expires, especially if you are above 40 years of age. Some RTOs require Form 1A at every renewal cycle. Commercial vehicle (LMV/HMV) licence holders must submit Form 1A at every renewal regardless of age.

Our Form 1A is accepted at RTOs across most Indian states, including Delhi (DL), Telangana (TS), Andhra Pradesh (AP), Uttar Pradesh (UP), Gujarat (GJ), Rajasthan (RJ), Punjab (PB), West Bengal (WB), Haryana (HR), Kerala (KL) and Madhya Pradesh (MP). Four states — Karnataka (KA), Maharashtra (MH), Tamil Nadu (TN) and Odisha (OD) — require Form 1A to be filed by a doctor empanelled on the Parivahan/Sarathi portal, and we do not currently serve those states. If you are applying at an RTO in one of them, you will need a portal-empanelled doctor instead. The certificate includes your state's RTO format details.

No clinic visit, no hospital appointment, no waiting. The complete process is done from your phone or laptop.

Yes. By default you receive a digitally signed PDF on WhatsApp and email — which most RTOs accept as a printout. If your specific RTO requires an original stamped hard copy, we offer physical delivery by India Speed Post to your address within 8–10 business days at a small additional charge.
