College Asked for a Medical Certificate for Leave? Here's Exactly What to Submit

Published: 20 February 2026  ·  6 min read

What Type of Medical Certificate Does a College Accept?

Indian colleges and universities accept medical certificates for leave, exam absence, or attendance condonation if the certificate is issued by a registered medical practitioner — meaning a doctor with an active NMC (National Medical Commission) registration. The certificate must be on clinic letterhead and contain the following elements:

  • Student's full name and age
  • Date of medical examination / consultation
  • Diagnosis or reason for inability to attend college
  • Recommended leave period (from date to date)
  • Doctor's full name, MBBS/MD qualification
  • Doctor's NMC registration number — required by most college authorities
  • Doctor's signature and clinic stamp on official letterhead

Common Situations Where Students Need Medical Certificates

Class / Lab Attendance ShortageMost colleges require 75% attendance. A medical certificate can be submitted to the attendance committee for condonation of absence due to illness.
Exam AbsenceIf you miss a mid-term, end-term, or internal exam due to illness, a medical certificate supports your application for re-examination or supplementary exam.
Assignment / Submission ExtensionFaculty usually accept a medical certificate as grounds for extending assignment submission deadlines.
Hostel Leave ExtensionHostel wardens typically require a medical certificate if you need to remain at home beyond the sanctioned leave period due to illness.
Admission Medical CertificateEngineering, medical, and management colleges require a fitness certificate during admission. This is a separate fitness certificate, not a sick leave certificate.
Sports Event AbsenceIf you miss a mandatory sports or NCC activity due to illness, a medical certificate is required for attendance to be excused.

Do Colleges Accept Online Medical Certificates?

Yes. Online medical certificates issued by NMC-registered MBBS/MD doctors via telemedicine under the MoHFW Telemedicine Practice Guidelines 2020 (GSR 225(E), Gazette of India, 25 March 2020) are accepted by all private colleges, deemed universities, and most state universities across India. The certificate must contain the doctor's NMC registration number, which the college can verify independently at nmc.org.in.

Exception: A small number of autonomous government colleges (notably some government medical colleges and IITs for certain specific purposes) may require a certificate from a government hospital or a college-empanelled doctor for specific academic purposes. Check your college's specific leave/attendance policy notice. For the vast majority of private colleges and state universities, our online certificate is accepted.

Frequently Asked Questions

Can I get a backdated medical certificate for college leave?

A doctor can issue a certificate stating that you were suffering from a specific illness during a past period, based on your reported medical history during the online consultation. This is called a retrospective or backdated certificate and is medically and legally valid when the doctor's assessment of your reported symptoms supports the diagnosis. The certificate will be dated on the day of consultation but will mention the period of illness. Most colleges accept retrospective certificates if the illness period is recent (within the last 2–3 weeks). For longer periods, an in-person doctor visit with documented treatment history is advisable.

What attendance percentage can be condoned with a medical certificate?

Most Indian colleges and universities allow attendance condonation for genuine illness with a medical certificate subject to the following typical limits: attendance must not fall below 65% even with condonation (some colleges allow up to 60% with medical grounds); the number of days condoned typically cannot exceed 10–15% of total working days in the semester; and the medical certificate must cover the specific dates of absence. Each college has its own attendance condonation policy — check your college's academic regulations. Our medical certificate clearly specifies the recommended leave dates, which the college's attendance committee uses to evaluate the condonation request.
