The short answer: Form 1A has two halves. You complete the applicant details — name, address, date of birth, and identifying particulars. A registered doctor completes the medical section, recording examination findings and signing with their registration number. You cannot fill in the doctor's half yourself, and a certificate without the doctor's registration number will not be accepted by the RTO.
The Two Halves of Form 1A
Form 1A is issued under Rule 5 of the Central Motor Vehicles Rules 1989. Unlike Form 1, which is entirely self-declared, Form 1A is a shared document:

Never fill in or pre-write the doctor's section. A certificate completed by anyone other than the examining practitioner is invalid, and RTOs verify the registration number against the medical register.
Your Half — Filling the Applicant Section
Full name
Write it exactly as it appears on the identity document you are submitting with the licence application. If your Aadhaar spells your name in full, do not abbreviate it to initials here. Name mismatches are the most common cause of a certificate being sent back at verification.
Address
Use the address supported by the proof you are submitting to the RTO — not simply where you currently live. The RTO checks the certificate against your application documents.
Date of birth
Copy it from your age proof. This field is significant because age determines whether Form 1A is mandatory in the first place.
Identifying marks
Where the form asks for identification particulars, complete them as recorded on your other licence documents so the details are consistent across the application.
Your signature
Sign in the same style as the signature on your supporting documents. Sign only after your details are filled in — never on a blank form.
The Doctor’s Half — What Gets Examined
The medical section covers the health parameters relevant to safe driving. The examining doctor assesses and records findings, typically covering:
- Vision — including distance vision and whether correction by spectacles or lenses is required.
- Colour vision, relevant to reading traffic signals.
- Hearing.
- Neurological history — epilepsy, sudden loss of consciousness, giddiness.
- Limb function and any physical impairment affecting vehicle control.
- General fitness to operate a motor vehicle.
Wearing spectacles is not a barrier. The certificate records that vision is corrected, and licences are routinely issued with a corrective-lenses condition noted.
Who Can Sign Form 1A
Form 1A must be completed by a registered medical practitioner. The certificate carries the doctor's signature, registration number, and seal, and the RTO can verify that number against the medical register.

A certificate signed by a practitioner without a verifiable registration number will be rejected. This is the single most important thing to check before you accept a completed Form 1A from anyone.

- Sign in to the Parivahan Sarathi portal and open your application.
