Why the Two Get Confused
Three reasons, and knowing them saves a wasted trip:
- The names are nearly identical. A single letter separates them, and RTO staff often say just "Form 1" in conversation when they mean either.
- They are frequently submitted together. Many applications need both, so people assume they are two halves of one document.
- "Medical certificate" is used loosely. When someone says you need a medical certificate for your licence, they nearly always mean Form 1A — but the phrase itself does not distinguish them.

Can You Submit Form 1 Instead of Form 1A?
