What the Form 1A Assessment Covers

The Form 1A medical assessment evaluates the following health parameters. An NMC-registered doctor must certify each area:

1. Vision Assessment (6/6 Standard)

This is the most critical component. The doctor tests visual acuity in both eyes — the standard requirement is 6/6 in the better eye (or corrected to 6/6 with glasses). Night vision, colour vision (ability to distinguish red and green traffic signals), and field of vision are also assessed. Applicants who cannot meet the vision standard even with corrective lenses may have their licence restricted or refused.

2. Hearing

The driver must be able to hear a whispered conversation at approximately 6 metres. Applicants who are severely hearing-impaired but can drive safely with aids may receive a restricted licence at the RTO's discretion.

3. Cardiovascular Health

The doctor checks blood pressure, heart rate, and asks about a history of heart attacks, arrhythmias, or uncontrolled hypertension. Recent major cardiac events (within 6 months) can be grounds for deferral pending specialist clearance.

4. Neurological / Epilepsy History

Applicants must declare whether they have a history of epilepsy, blackouts, or seizures. Under the Central Motor Vehicles Rules, a person with active epilepsy is not eligible for a commercial vehicle licence. For private vehicle licences, applicants who have been seizure-free for 2 years on medication may qualify at the RTO's discretion.

5. General Fitness

The doctor also assesses for diabetes (particularly insulin-dependent diabetes with hypoglycaemia risk), significant physical disabilities that could impair vehicle control, and alcohol or substance dependence. These do not automatically disqualify a driver but are noted in the certificate.

What Happens If Your Form 1A Is Rejected by the RTO?

Genuine medical rejections — where the examining doctor certifies that you do not meet the fitness standards — are rare and have nothing to do with the certificate provider. If, however, your RTO raises an administrative objection to the certificate format, here is what to do:

Validity: How Long Does Form 1A Last?

A Form 1A medical certificate is valid for one year from the date of issue. If your driving licence renewal application is processed within one year of the certificate date, the same certificate can be used. If your application is delayed beyond one year, you will need a fresh Form 1A.
