How to Download the Blank Form 1A
If you want to take the offline route and get Form 1A filled by a doctor in person, you will need the blank form first. Here is how to download it from the official Parivahan portal, and what the Sarathi screens look like when you submit the signed copy.
Downloading it from parivahan.gov.in
- Open the Parivahan downloadable forms page for Central Motor Vehicles Rules (CMVR) forms.
- Locate Form 1A — Medical Certificate in the list, or go straight to the official FORM-1A.pdf.
- Download the PDF. The blank form is free — no login or registration is required.
- Print it, then carry it to a registered medical practitioner who completes, signs, and stamps the medical sections.
- Upload the signed PDF back on the Sarathi portal with your driving licence application, or hand it in at the RTO counter.
On the forms page, Form 1A appears under the Driving License category, listed as "Form-1A" with the description "Medical certificate" (a 14 KB PDF).
Important: only your basic details — name, address, date of birth — may be filled in by you. The medical sections must be completed and signed by a registered medical practitioner. A blank or self-filled medical section will be rejected at the RTO.

Submitting the signed form on Sarathi
Once a doctor has signed your Form 1A, you attach it to your driving licence application on the Parivahan Sarathi portal. Here is what each stage looks like.
Step 1
Open sarathi.parivahan.gov.in and select the state where you are applying, from the "Select State Name" dropdown.
Step 2
From the Driving Licence services menu, choose "Apply for DL Renewal" — or the service that matches your application.
Step 3
Enter your driving licence number, date of birth and the captcha to load your details, then continue to the document upload step where Form 1A is attached.

Certificate Legally Valid?
Yes — completely. The Ministry of Health & Family Welfare notified the Telemedicine Practice Guidelines 2020 (Gazette of India, GSR 225(E), 25 March 2020), which legally authorise NMC-registered doctors to issue Form 1A medical certificates after an online assessment.
Our Form 1A certificates carry the doctor's full name and NMC registration number — the same details required on a clinic-issued Form 1A. RTO officers can verify the doctor's credentials independently on nmc.org.in.

Where to Submit Form 1A & What It Costs
Your ₹999 covers the Form 1A medical certificate only. The RTO charges its own statutory licence fees separately, under Rule 32 of the Central Motor Vehicles Rules 1989.

States we do not serve: RTOs in Karnataka, Maharashtra, Tamil Nadu and Odisha require the issuing doctor to be empanelled on the Parivahan/Sarathi portal — NMC registration alone is not sufficient there. We do not currently issue Form 1A for those four states; if you are applying at an RTO in one of them, you will need a portal-empanelled doctor instead.

RTO verification
RTOThe Regional Transport Office checks the certificate and can verify the doctor’s NMC registration number independently at nmc.org.in. Some states also require an in-person biometric or document-verification visit.
Who uploads the certificate — and why it varies by state
Some states operate an empanelment system, where a doctor registered on the Parivahan/Sarathi portal can file Form 1A directly against your application, and you never handle the file. In the rest of India the applicant uploads the signed certificate themselves.
How we work: our doctors issue your Form 1A and send the signed PDF to you — you upload it on Parivahan yourself. We do not file it directly against your application in any state. Karnataka, Maharashtra, Tamil Nadu and Odisha require a portal-empanelled doctor, so we do not currently serve those four states.
After you submit: what to expect
Once the RTO accepts your documents, a driving licence application is typically processed within a few weeks and the licence is despatched by post, though timelines vary considerably by state and by RTO workload. Your Sarathi application status page is the authoritative source — it also tells you whether an in-person visit is still outstanding. Note that Form 1A itself is valid for six months from issue, so submit it well inside that window.
