What is the difference between a sick leave certificate and a fitness certificate?

They state opposite things. A sick leave certificate says you are medically unfit for duty and need rest for a specified period, and you submit it to excuse an absence that has happened or is happening. A medical fitness certificate says you are healthy and fit to undertake an activity, and you submit it before starting something: joining a job, enrolling in a college, applying for a driving licence, or competing in a sporting event. A sick leave certificate covers dates; a fitness certificate certifies your condition on the day you were examined.

Can I get a medical certificate without visiting a hospital or clinic?

Yes. Under the MoHFW Telemedicine Practice Guidelines 2020, a registered medical practitioner may consult a patient remotely and issue documentation arising from that consultation, including medical certificates. You describe your symptoms or the purpose of the certificate, an NMC-registered MBBS or MD doctor assesses you online, and the signed certificate is delivered to your WhatsApp and email. No clinic visit, no travel, and no waiting room. An in-person examination is still required where the certificate depends on measured vitals, laboratory tests, or an examination by a specified or empanelled doctor.

Is an online medical certificate accepted by employers?

Generally yes. What HR teams check is that the certificate comes from a registered medical practitioner, carries the doctor’s NMC registration number (verifiable at nmc.org.in), names you as recorded in their files, states the period and reason, and is signed on clinic letterhead. Validity comes from the doctor’s registration rather than the setting of the consultation, so a telemedicine certificate meeting those points stands on the same footing as one collected in a clinic. Individual employers can still have their own documentation policies for long absences.

What is the standard format of a medical certificate in India?

There is no single statutory format for a general medical certificate, but an accepted one carries: the clinic or hospital letterhead; the patient’s name, age and gender; the date of consultation and the date of issue; the medical condition in general clinical terms; the recommended rest period or fitness declaration; and the doctor’s full name, MBBS or MD qualification, NMC registration number and signature. Form 1A for driving licences is the exception, since it has a prescribed government format under the Central Motor Vehicles Rules.

What documents are required to get a medical certificate?

For a standard sick leave or fitness certificate, usually just your name, age and the purpose the certificate is for. Photo ID is helpful and is asked for with Form 1A driving licence certificates. Prior medical reports are not required for a routine assessment, though sharing them helps if you have an ongoing condition relevant to the certificate. Certificates that depend on test results, such as some visa and employment medicals, require those reports or an in-person examination.
