A standard medical certificate in India must include specific elements to be legally valid — especially when issued under telemedicine guidelines. Below are the two most common formats.
1. Sick Leave Certificate Format
This is to certify that Mr./Ms. [Patient Name], aged [Age] years, was examined by me on [Date].
He/She is suffering from [Diagnosis / Illness] and is advised complete rest for [X] days from [Date] to [Date].
He/She is unfit to attend duties/school/college during this period.
2. Medical Fitness Certificate Format
This is to certify that Mr./Ms. [Patient Name], aged [Age] years, Son/Daughter of [Father's Name], was clinically examined by me on [Date].
On examination, he/she is found to be in good health and mentally and physically fit to join [Institution/Organisation/Purpose].
He/She does not suffer from any communicable disease or any condition that would prevent him/her from performing the duties associated with the position/programme.
Mandatory Elements for a Valid Medical Certificate
- Doctor's full name and qualifications (MBBS/MD minimum)
- NMC registration number — verifiable at nmc.org.in
- Patient's name and age
- Date of examination
- Diagnosis or purpose (illness for sick leave; fitness declaration for fitness certificate)
- Rest period (for sick leave) or fitness declaration (for fitness certificate)
- Doctor's signature (digital signature is valid under IT Act 2000)
- Clinic/hospital name and contact
