How Many Days of Sick Leave Can You Get with a Medical Certificate? (India 2026)

The most common questions we get: "Can I get a certificate for 2 days?", "Can an online doctor give me 5 days?", "What's the maximum sick leave I can get from a medical certificate?" This guide answers all of these based on Indian law, NMC guidelines, and actual doctor practice.
Online NMC-registered doctors issue sick leave certificates for 1 to 7 days per consultation. The duration depends on your illness — not on what you ask for. The doctor decides based on the nature of the condition, symptom severity, and standard recovery timelines.
Sick Leave Duration by Condition — Quick Reference Table
| Condition | Typical Duration | Max via Telemedicine | Notes |
|---|---|---|---|
| Common cold / URTI | 1–3 days | 3 days | Self-limiting; usually clears in 3–5 days |
| Viral fever | 2–5 days | 5 days | Most common sick leave reason in India |
| Dengue fever | 5–7 days | 7 days | Requires rest until fever resolves |
| Typhoid fever | 5–7 days | 7 days | Antibiotic treatment + rest required |
| Gastroenteritis / food poisoning | 2–3 days | 3 days | Dehydration-related weakness included |
| Migraine | 1–2 days | 2 days | Acute episode with photosensitivity |
| Back pain (acute lumbar) | 2–4 days | 4 days | Muscle strain or acute spasm |
| Flu / influenza | 3–5 days | 5 days | Body ache and fatigue may persist |
| Sinusitis | 2–4 days | 4 days | With antibiotic treatment prescribed |
| UTI (urinary tract infection) | 2–3 days | 3 days | Antibiotic treatment + rest |
| Menstrual pain (dysmenorrhoea) | 1–2 days | 2 days | Acute pain episode |
| Chickenpox | 7 days | 7 days | Isolation until lesions crust over |
What Determines How Many Days the Doctor Gives?
The NMC-registered doctor determines sick leave duration based on:
- Nature of the illness: Standard recovery timelines for each condition
- Symptom severity: Mild viral fever = 2 days; severe fever with vomiting = 4–5 days
- Onset date: How many days you have already been unwell
- Work nature: Physical jobs (on-site) may warrant more rest than desk jobs
- Any lab reports: A positive dengue NS1 or typhoid test changes the clinical picture
The doctor does not simply give you however many days you ask for. Requesting 10 days for a common cold will result in a certificate for 2–3 days — the medically appropriate duration. This is what makes our certificates credible and accepted by HR departments.
Company-Specific Rules: When Do You Need a Certificate?
IT Companies & MNCs
Most require a certificate for 2+ consecutive days. Some (like Infosys, TCS) allow 3 days self-certification.
Central Government (CCS Rules)
Medical certificate required for sick leave of any duration if applying for Earned Leave on medical grounds.
State Government
Similar to CCS — certificate required for sick leave applications beyond self-certification limits (varies by state, usually 3 days).
Banks (Nationalised)
SBI and PSU banks typically require certificate for 2+ consecutive days. Most private banks have the same policy.
Schools & Colleges
Schools typically require certificate for 3+ days. Colleges for 3+ days, though attendance condonation requires specific dates.
PSUs (ONGC, BHEL, etc.)
Generally follow CCS-adjacent rules — certificate from registered medical practitioner required for all sick leave sanctions.
Can I Get a Sick Leave Certificate for Just 1 Day?
Yes. Our doctors issue certificates for 1 day when the medical need is present — for example, an acute migraine that prevents working for a single day, or a severe one-day episode of food poisoning. However, most employers only require a medical certificate for 2 or more consecutive days. For a single day, check your company's HR policy — you may be able to self-certify.
What If I Need More Than 7 Days?
For conditions requiring more than 7 days of rest — post-surgery recovery, serious infections, or chronic condition flare-ups — the doctor may:
- Issue a certificate for 7 days initially and recommend a follow-up consultation for extension
- Recommend an in-person visit to a specialist who can assess and certify the extended need
- Issue a second certificate after a follow-up telemedicine consultation
Government employees and PSU staff requiring extended sick leave (more than 30 days) will typically need a Medical Board certificate — which requires in-person assessment at a government hospital.
